No. Alternatively, you may file a Workers’ Compensation claim through your doctor using Oregon Form 827. With the help of your physician, the form is completed jointly. The physician, clinic or hospital is then obligated to report job-related injuries to the Workers’ Compensation insurance company within 3 business days.
