How do I file a Workers’ Compensation injury claim?
First, notify your employer immediately of your injury. While you are not required to file a claim, if you choose to do so, ask your employer for Oregon form 801, and complete it. Once notified of the injury, the employer is obligated to file the form with the Workers’ Compensation insurer within 5 days.
